同时进行无痛胃镜和肠镜检查的临床价值【摘要】目的探讨同时进行无痛胃镜、无痛肠镜检查对上消化道疾病诊断与治疗的优越性。方法随机把病人分为无痛组和对照组，无痛组又分为A组麻醉后由两位医生同时进行胃镜肠镜检查；B组在同一诊室麻醉后先进行胃镜检查再进行肠镜检查。C组麻醉后先进行胃镜检查，再送到肠镜室进行肠镜检查。对照组无麻醉下先进行胃镜检查，略休息后再到肠镜室进行肠镜检查。分析比较各组检查的情况。结果(1)无痛各组患者感觉满意均达100％，对照组满意55．6％，无痛组满意度显着高于对照组；(2)各组操作时间及用药量：A组时间最短，用药最少，C组用药最多，对照组检查时间最长，各组用药量及检查时间差异有统计学意义；(3)无痛各组患者血压升高、心率增快均较对照组显着减少，但出现血氧饱和度下降较对照组多。结论同时进行无痛胃镜、肠镜检查是安全可行的诊治手段。【关键词】无痛胃镜；无痛结肠镜；并发症Abstract：ObjectiveToexploretheadvantageofsimultaneousapplicationofpainlessgastroscopyandpainlessenteroscopyinthediagnosisandthetreatmentofupperdigestivetractdiseases.MethodsThepatientsenrolledintothestudyweredividedintofourgroupsatrandom.InGoupA,thepatients(n=23)underwentgastroscopyandenteroscopyatthesametimeafterbeinganaesthetised.InGroupB,thepatients(n=19)underwentgastroscopyandenteroscopysuccessivelyinthesameinspectionroomundertheconditionofanaesthesia.InGroupC,gastroscopywasperformedonthepatients(n=25)inGastroscopicRoomundertheconditionofanaesthesia,andthenenteroscopywasperformedinEnteroscopicRoom.InGroupDactingasacontrolgroup,gastroscopywasperformedonthepatients(n=18)inGastroscopicRoomwithoutanaesthesia,andafterawhileenteroscopywasperformedinEnteroscopic(1)Thefeelingsatisfactoryrateofanyofthepainlessgroupswasupto100%,andthecounterpartofthecontrolgroupwasdownto%.Theformerwassignificantlyhigherthanthelatter.(2)ItwasinGroupAthattheshortesttimeandtheleastdrugdosagewereused.InGroupC,thelargestdrugdosagewasused,andinGroupDthelongestinspectiontimewasspent.Thereweresignificantdifferencesindrugdosageandinspectiontimeamongthegroups.(3)Increaseinbloodpresureorheartratewaslesscommoninthepainlessgroups,whilethedecreaseinbloodoxygensaturationwasmorecommon.ConclusionThesimultaneousapplicationofpainlessgastroscopyandenteroscopyisasafeandreliableinspectionapproach.Keywords：painlessgastroscopy;painlessenteroscopy;complication胃肠镜是消化道疾病诊治的主要方法，由于胃肠镜检查有一定的痛苦，部分患者因拒绝检查而延误诊断与治疗。目前无痛胃镜、无痛肠镜检查临床已经广泛开展，但同时进行无痛胃镜无痛肠镜检查尚未见报道。本组总结同时进行无痛胃镜、肠镜检查的患者67例，与常规同时进行胃肠镜检查的18例患者进行比较，探讨其在临床的应用价值。1资料与方法1．1一般资料2003年9
