右美托咪定对高原患者心率的影响 Abstract Rightventricularimpairedfunctionandhypoxemiaareacommonresultofhypobarichypoxiainhigh-altituderegions,whichcanleadtoanincreaseinheartrateasacompensatorymechanism.TheadministrationofDexmedetomidine(DEX)asaselectiveα2-adrenoceptoragonistmayhaveabeneficialeffectbydecreasingtheheartrateofpatientsinhighaltituderegions.Inthisarticle,wereviewtheeffectsofDEXonheartrateinhigh-altitudepatients. Introduction High-altituderegionsarecharacterizedbylowatmosphericpressureandreducedoxygenlevelsintheair.TheseareasarefoundinregionssuchasTibet,Andes,andtheHimalayas,wherepeopleareexposedtohighaltitudeforprolongedperiods.Duringexposuretohypobarichypoxia,theconcentrationofoxygeninthebloodandthepressureinthealveolidecrease,leadingtovariouscardiovascularcomplicationssuchasincreasedheartrate,pulmonaryhypertension,anddecreasedcardiacoutput.Elevatedheartrateisawell-documentedcompensatorymechanisminresponsetohypoxia-inducedlowoxygenlevels.Hypoxia-inducedincreasesinheartratecanhaveadverseoutcomessuchasincreasedmyocardialoxygendemand,theriskofcardiacarrhythmias,andheartfailure. Rightventricularimpairedfunctionandhypoxemiaareacommonresultofhypobarichypoxiainhigh-altituderegions,andthesefactorscancontributetoanincreaseinheartrateduetoacompensatorymechanism.Theuseofpharmacologicalinterventioncanhelptomitigatetheseeffectsandimprovetheoutcomesofpatients,particularlythosewithcardiovasculardiseases. Dexmedetomidine(DEX),aselectiveα2-adrenoceptoragonist,isacommonlyusedsedativeincriticallyillpatients,providinganalgesiceffectsanddecreasingagitation,delirium,andtheneedformechanicalventilation.Itseffectsonreducingheartratecouldbebeneficialforindividualswithcardiovascularcomplicationsinhigh-altituderegions.
