全膝关节初次置换术后行翻修术原因分析及疗效随访的中期报告 摘要:目的:探讨全膝关节置换术后翻修术的原因及中期疗效。方法:回顾性分析2007年1月至2012年12月在本院行全膝关节初次置换术的患者资料,收集翻修的原因、手术方式、手术时间、手术并发症等资料。对翻修前后的KneeSociety评分、膝关节弯曲(°)、延伸(°)以及手术并发症等指标进行比较。结果:共有45例(45膝)行膝关节置换术后翻修手术,翻修的患者中,14例(31.1%)为非骨溶性假体松动(asepticloosening),14例(31.1%)为感染(infection),17例(37.8%)为其它。平均手术时间为3.8±1.9小时,手术时间延长率为21.3%。手术后发生的并发症包括:切口愈合不良4例(8.9%)、术后膝关节感染5例(11.1%)、深静脉血栓形成2例(4.4%)。手术后平均随访34.5个月(12~74个月),与术前相比,KneeSociety评分、膝关节弯曲(°)和延伸(°)均有明显改善(P<0.05)。结论:全膝关节置换术后翻修术是一种有效治疗手段。非骨溶性假体松动和感染是翻修的主要原因,对于复发性感染或非骨溶性假体松动的患者,应考虑翻修手术。翻修手术应尽量避免并发症的发生,术后持续随访可评价疗效并早期发现并发症。关键词:膝关节置换术;翻修术;原因;疗效随访;中图分类号:R687.4文献标志码:A Abstract:Objective:Toinvestigatethereasonsandmid-termefficacyofrevisionsurgeryaftertotalkneearthroplasty.Methods:RetrospectiveanalysisofthedataofpatientswhohadundergoneprimarytotalkneearthroplastyinourhospitalfromJanuary2007toDecember2012,andcollecteddataonthereasonsforrevision,surgicalmethods,surgicaltime,surgicalcomplications,etc.ComparetheKneeSocietyscores,kneeflexion(°),extension(°),andsurgicalcomplicationsbeforeandafterrevision.Results:Atotalof45cases(45knees)underwentrevisionsurgeryafterkneereplacement.Amongtherevisionpatients,14cases(31.1%)wereasepticloosening,14cases(31.1%)wereinfected,and17cases(37.8%)wereothers.Theaverageoperationtimewas3.8±1.9hours,andthetimeextensionratewas21.3%.Thecomplicationsaftersurgeryincluded:poorwoundhealingin4cases(8.9%),kneejointinfectionin5cases(11.1%),anddeepvenousthrombosisin2cases(4.4%).Theaveragefollow-uptimeaftersurgerywas34.5months(12-74months).Comparedwithbeforesurgery,theKneeSocietyscores,kneeflexion(°),andextension(°)showedsignificantimprovement(P<0.05).Conclusion:Revisionsurgeryaftertotalkneearthroplastyisaneffectivetreatmentmethod.Asepticlooseningandinfectionarethemainreasonsforrevision.Forpatientswithrecurrentinfectionorasepticloosening,revisionsurgeryshouldbeconsidered.Theoccurrenceofcomplicationsshouldbeavoidedasmuchaspossibleforrevisionsurgery,andcontinuousfollow-upaftersurgerycanevaluatetheefficacyanddiscovercomplicationsea
