Introduction: Endometriosisaffectsmillionsofwomenworldwideandisassociatedwithsignificantpain,infertility,andreducedqualityoflife.Althoughseveraltheorieshavebeenproposed,includingretrogrademenstruation,immunedysfunction,geneticpredisposition,andhormonalimbalances,theexactcauseofendometriosisremainselusive.Emergingevidencesuggeststhatimmunologicalfactorsplayacrucialroleinthedevelopmentandprogressionofendometriosis.sHLA-Ghasbeenimplicatedinimmunemodulationandtoleranceduringpregnancyandvariouspathologicalconditions.However,itsroleinendometriosisremainsunclear. Methods: Atotalof100patientsdiagnosedwithendometriosisand100healthycontrolswereenrolledinthisstudy.SerumsampleswerecollectedandanalyzedforsHLA-Gexpressionbyenzyme-linkedimmunosorbentassay(ELISA).Clinicaldataincludingage,stageofdisease,painscores,andfertilitystatuswererecordedandcorrelatedwithsHLA-Glevels. Results: TheresultsrevealedsignificantlyhigherlevelsofsHLA-Gintheserumofpatientswithendometriosiscomparedtohealthycontrols(p<0.001).Moreover,sHLA-Glevelswerepositivelycorrelatedwithdiseasestage(p<0.01)andpainscores(p<0.05).However,nosignificantassociationwasfoundbetweensHLA-Glevelsandfertilitystatus. Discussion: TheoverexpressionofsHLA-Ginendometriosispatientssuggestsitspotentialroleinthepathogenesisofthedisease.sHLA-GcanactasanimmunemodulatorbysuppressingcytotoxicTcellactivity,naturalkillercellfunction,andpro-inflammatorycytokineproduction.Thisimmunosuppressiveeffectmaycontribut
