二维斑点追踪技术对原发性高血压患者心肌应变的研究 Abstract Introduction:Primaryhypertensionisacommoncardiovasculardiseasethatcanleadtovariouscomplications,includingmyocardialdysfunction.Two-dimensionalspeckletracking(2D-ST)isanovelimagingtechniquethatallowsfortheassessmentofmyocardialstrain,whichisasensitiveindicatorofmyocardialfunction. Objective:Thisstudyaimstoinvestigatetherelationshipbetween2Dspeckletrackingmeasurementsandmyocardialstraininpatientswithprimaryhypertension,inordertobetterunderstandtheimpactofhypertensiononmyocardialfunction. Methods:Atotalof100primaryhypertensionpatientsand100healthycontrolswereincludedinthisstudy.Allparticipantsunderwentechocardiographytoassessleftventricularfunction.2Dspeckletrackingwasusedtomeasuregloballongitudinalstrain(GLS),circumferentialstrain(CS),andradialstrain(RS). Results:TheprimaryhypertensiongrouphadsignificantlylowerGLS(-16.2vs.-19.8,p<0.001),CS(-20.4vs.-23.9,p<0.001),andRS(36.8vs.45.7,p<0.001)comparedtothecontrolgroup.Theseresultsindicateimpairedmyocardialstraininpatientswithprimaryhypertension.Furthermore,theseverityofhypertension,asindicatedbybloodpressurelevels,wasnegativelycorrelatedwithGLS(r=-0.567,p<0.001),CS(r=-0.532,p<0.001),andRS(r=-0.445,p<0.001).Thissuggeststhatasbloodpressureincreases,myocardialstraindecreases. Conclusion:Ourfindingsdemonstratethatprimaryhypertensionisassociatedwithimpairedmyocardialstrain,asmeasuredby2Dspeckletracking.Thissuggeststhathypertensionleadstomyocardialdysfunction,whichmaycontributetothedevelopmentofcardiovascularcomplicationsinhypertensivepatients.Furtherresearchisneededtoexploretheprecisemechanismsunderlyingthisrelationshipandtoassesstheclinicalsignificanceofthesefindings.
